Register with this Practice

Before filling out a registration form, check you are in our catchment area.

Patients are registered with the Practice, not with an individual GP.  You can at any time express a preference for a particular doctor, for either or all of your medical needs or on a case by case basis.  However, not all of the doctors based within the Practice provide all services, furthermore if requesting specific doctors, they  may not be immediately available.

To Register

You may wish to print off and complete your registration form or complete the editable PDF document. This can be emailed to gram.haddoclinical@nhs.scot.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. Please take your competed form along with your registration form or email them both together.

• fill out the registration form and the new patient questionnaire form and email them to gram.haddoclinical@nhs.scot or return them to the practice
• visit reception and collect a paper registration form

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
