{"id":1132,"date":"2026-07-29T11:56:57","date_gmt":"2026-07-29T10:56:57","guid":{"rendered":"https:\/\/surgeryweb-scot.org.uk\/n30030\/?page_id=1132"},"modified":"2026-07-29T11:56:57","modified_gmt":"2026-07-29T10:56:57","slug":"sharing-information","status":"publish","type":"page","link":"https:\/\/surgeryweb-scot.org.uk\/n30030\/sharing-information\/","title":{"rendered":"Sharing Information"},"content":{"rendered":"<p><b>Key Information Summary (KIS)<\/b>\u00a0has been designed to support patients who have complex care needs or long term conditions.<\/p>\n<p>KIS allows important information to be shared with health care professionals in unscheduled care.\u00a0 Information is shared with patient consent and KIS will be available for users within NHS24, out of hours organisations, A &amp; E departments\u00a0plus hospital and pharmacy environments.<\/p>\n<p>KIS contains special information from the GP practice and also:<\/p>\n<ol>\n<li>Future care plans<\/li>\n<li>Medications and allergies<\/li>\n<li>Current situation, including main diagnosis and current issues<\/li>\n<li>Carer and support details including next of kin<\/li>\n<li>Information and recommended action for\u00a0out of hours\u00a0clinicians<\/li>\n<li>Your wishes<\/li>\n<\/ol>\n<p>In the future, KIS will also be used in scheduled care for patients with long-term conditions; for example, for those who regularly visit renal clinics and within the Scottish Ambulance Service.<\/p>\n<h3><strong>Information Governance \u2013 Data Protection<\/strong><\/h3>\n<p>Back Scanning of Paper GP Records in Garthdee Medical Group<\/p>\n<p dir=\"auto\">We are now starting the process of Back Scanning all\u00a0 paper GP Records that we hold.\u00a0 For more information, please <a href=\"https:\/\/surgeryweb-scot.org.uk\/n30030\/wp-content\/uploads\/2026\/07\/Privacy-Notice-Back-scanning-GP-Records-NEC.docx\">click\u00a0here<\/a>.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Key Information Summary (KIS)\u00a0has been designed to support patients who have complex care needs or long term conditions.
