This is because it can take a considerable time for us to receive your medical records. There is a printable version of this below; please select the correct one depending on what age you are, either 16 years old and over, or under 16 years old.<\/p>\n<p><a href=\"https:\/\/surgeryweb-scot.org.uk\/55874\/wp-content\/uploads\/2026\/08\/GPR-Form.pdf\">General Practice Registration Form<\/a><\/p>\n<p><a href=\"https:\/\/surgeryweb-scot.org.uk\/55874\/wp-content\/uploads\/2026\/08\/New-Patient-Questionnaire-Adult-2025.pdf\">New Patient Questionnaire &#8211; 16 and Over<\/a><\/p>\n<p><a href=\"https:\/\/surgeryweb-scot.org.uk\/55874\/wp-content\/uploads\/2026\/08\/New-Patient-under-16-Questionnaire-2016-NEW.docx\">New Patient Questionnaire &#8211; under 16<\/a><\/p>\n<p>All new patients are required to provide two acceptable forms of identity and residence in the UK prior to registration, one of which must be photographic (i.e. passport or driving licence). Other acceptable forms include council tax documents, rent book, utility bills, television licence, original marriage\/birth certificate (not recently issued), flight tickets to the UK (single journey) and bills for shipping of personal belongings to the UK.<\/p>\n<p>Entitlement to NHS care in Scotland is generally based on residency status not nationality.
