These include HGV medicals, driving applications and holiday cancellation forms.<\/p>\n<p>You will be informed of any costs prior to completion of any forms.\u00a0 Please ask the reception staff for further details (<strong>NB<\/strong>\u00a0we are unable to accept debit\/credit cards for payment).<\/p>\n<p>Our Doctors can provide a range of letters or reports including HGV Medicals, DVLA Reports, Power of Attorney&#8217;s, letters of simple fact or more complex letters. The charge of the letters and reports vary so please contact the surgery to find out the cost.<\/p>\n<p>Letters for Jury Exemption are free of charge.<\/p>\n<p>We cannot provide letters to support housing applications.
