The Health Board requires we collect proof of identity so please bring some suitable documents for each person registering.\u00a0 You will need to complete a Registration form in person, sign it and hand it in to our Reception desk.<\/p>\n<p>You will be asked to complete a New Patient Questionnaire which asks for some information about you such as any on-going\u00a0medical conditions and any current medication you may be taking. This helps the doctors provide your care whilst we await transfer of your medical records from your previous practice.<\/p>\n<p>If you move out of the practice area you must register as soon as you can with a new GP with a catchment area covering your new address.<\/p>\n<p>Medical treatment is available from the date of registration. Please contact reception for further information.<\/p>\n<p><a href=\"https:\/\/surgeryweb-scot.org.uk\/40224\/wp-content\/uploads\/2026\/08\/New-Patient-Health-QuestionnaireTHE-GREEN-MEDICAL-PRACTICE-February-2024.pdf\">New Patient Health Questionnaire<\/a><\/p>\n<p>If you prefer you can ask for a copy of the New Patient Health Questionnaire at our reception desk.<\/p>\n<h3>Temporary Registration<\/h3>\n<p>If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.<\/p>\n<p>You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.<\/p>\n<p>To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment.
